(1)供应商应进入“全国投资项目在线审批监管平台”中的工程咨询单位名录，且备案专业含公路，服务范围：全过程咨询。







(2)具有公路工程相关专业造价工程师职业资格证，公路工程相关专业包括公路工程、桥梁工程、公路与桥梁工程、交通土建、隧道(地下结构)工程、交通运输工程等专业；







(3)法定代表人资格证明书(法定代表人参会时提供) 。法定代表人资格证明书必须使用本文件规定 格式，否则投标无效。法定代表人授权委托书(法定代表人未参会时提供) 。如参会代表不 是法定代表人的，须附有授权委托书。法定代表人授权委托书必须使用本文件规定格式，否则投标无效。
