采用单一来源采购方式的原因及说明：本次项目采购智障、孤独症儿童康复救助项目（一），根据新疆维吾尔自治区人民政府文件“新政发【2018】70号”《关于建立残疾儿童康复救助制度的实施意见》；新疆维吾尔自治区（残疾人联合会、卫生和计划生育委员会、财政厅、民政厅、教育厅、人力资源和社会保障厅、工商行政管理局、审计厅、扶贫开发办公室）“新残字【2018】169号”关于印发《新疆维吾尔自治区残疾儿童康复救助实施办法》的通知；新疆维吾尔自治区（残疾人联合会、卫生健康委员会、民政厅、教育厅、工商行政管理局）“新残字【2018】188号”《关于印发自治区残疾儿童康复救助各类定点机构准入标准（试行）的通知》附件6智力残疾儿童康复救助定点康复训练机构准入标准（试行）第二条基本的康复训练场所与设施设备；附件7孤独症儿童康复救助定点康复训练机构准入标准（试行）第一条第2点规模与场地标准；阿克苏地区（残疾人联合会、卫生和健康委员会、财政局、民政局、教育局、医疗保障局、市场监督管理局、审计局、扶贫开发办公室）“阿地残字【2020】1号”关于印发《阿克苏地区残疾儿童康复救助工作实施办法的通知》附表1智障、孤独症儿童康复定点机构有温宿县残疾人康复中心和阿克苏地区启智儿童智力残疾康复中心；因上述两家定点机构都提交无康复训练场所情况说明，后阿克苏地区残疾人联合会下发文件“阿地残传【2021】1号”《关于增设地区智力残疾和孤独症儿童康复救助定点康复训练机构的通知》，该文件中提出经过申报智力残疾和孤独症儿童定点康复机构的3家康复机构实地考察、审核评定、现增设阿克苏市民办乐宝佳幼儿园为地区智力残疾、孤独症儿童定点康复训练机构。阿克苏市民办乐宝佳幼儿园更名为阿克苏地区乐宝佳智力残疾儿童康复中心（提供有更名申请及阿克苏地区残疾人联合会下发的更名申请批复）故推荐阿克苏地区乐宝佳智力残疾儿童康复中心为唯一供应商。
